Specialized Ministries Yearly Report Form (Non-Federal)

Beginning October 2011, all Non-Federal Chaplains and/or Pastoral Counselors will complete their Annual Reports through the CDM+ System.  Below are the steps you will need to follow in order to complete and submit your Annual Report.

1. Call Anne Marie Moyars at (888) 346-2631 ext. 2664 to receive a login e-mail address and password;

2. Once you have the login e-mail address and password please login to https://dhm.suran.com/minister/login;

3. At this point enter your login e-mail address and password then click login (the red button at the bottom on the right side);

4. Then choose “Chaplaincy”;

5. Then choose “Annual Chaplaincy form (click here)”;

6. Then choose “Create and Submit a new Annual Chaplaincy Form based on a blank form”;

7. Complete all sections of the form (1—5); and

8. Click submit

**Please note if you are a minister who already has access to the Search and Call Web MinPro System you will not need another login and password.  You will begin the process of submitting your Annual Report at step #2 from above using your Search and Call Web MinPro login and password.
