Minister Information Change Reporting

Overview

Welcome to the section of the DHM Web site where you can provide changes for clergy records in the Ministers Directory office, as well as determine the level of access to the church's Search and Call system for particular individuals.

For Local Clergy Submitting Their Own Relocation Information

If you are a Disciples minister, or Qualified UCC Ministerial Partner, who has recently:

  • accepted a new call to a position of ministry,
  • left a previous call but not yet submitted your relocation information, or
  • obtained new contact information (new home address, phone number, email address, etc.) 

...please submit a Ministerial Information Change Form to the Office of Search and Call. 

To Set-up Initial Regional Staff Authorization Account

If you are currently a Regional Staff Person but do not yet have an account on DHM's Web site as a Regional Staff Person, you can Register An Accountwithin the DHM Web site.

Once you have registered your personal account, please also submit a Regional Staff Authorization Form to notify DHM staff that you need access to the clergy forms.

For Authorized Regional Staff, Not Currently Logged In

If you already have a Regional Staff Person account on DHM's Web site, but are not logged in, you will be prompted to log in when clicking on the Ministerial Credentialing Forms link.

For Regional Staff Already Logged In

If you represent a Christian Church (Disciples of Christ) regional office, are authorized to submit clergy forms, and are currently logged in to the DHM Web site as a Regional Staff Person -- you may access these Ministerial Credentialing Forms by clicking on the side bar button.


 

Prayer for our ministers in ministry and transition

O God whose care for us all is bound together with a deep and intimate grace, we give thanks for the ways You guide each of us throughout our lives of vocation and mission.

For those ministers whose present days are lived amidst the ongoing care of a congregation to which they’ve offered pastoral leadership for some time, or in another institution wherein their presence has been like a healing salve upon the spiritual wounds of other children of God, we ask for Your continued guidance and wisdom in their ways of vocation.

For those who’ve heard Your call toward new ministry, and have entered into a searching process of discernment, but have yet to affirm where the future will lead, we ask for restful patience and prayerful attentiveness to Your lead. We request an openness in their seeking to the fullest possibilities of new ministry. We ask, also, for Your gifts of responsible diligence -- that these ministers in transition will discover a next authentic call that makes the best use of Your gifts in their lives. And, we pray for a deep, deep trust that You are, indeed, at the heart of their journey -- as they slowly disengage from one ministry and continue to search for the next community in which their pastoral leadership can abide.

We also pray for those who’ve recently affirmed a next call to serve You amidst Your people and world. Whether they are in their first days and weeks of a ministry that is new to them, or yet to begin that which has already been decided, we pray You will care for them as they seek to establish new relationships of caring leadership, and let go of that which is no more. May these ministers of Your church know patience and wisdom in their lives, that they may help Your people journey into celebrative possibilities of vitality and hope. May their care help move Your people into vibrant new transforming realities. May Your care fill them with enough stamina to thrive in their personal and communal lives, even amidst the challenges that are a part of every ministry in this world.

For these things we pray to you, even as we each open our hearts and minds to that which You might have to say to us, in the abundance of your grace throughout creation. Amen.
