This form is ONLY for use to initiate the Association process between existing and/or new clergy records in Regional and the General offices. This form can NOT be used to provide a minister with access to the Search And Call system, nor designate any credentials for a minister.

Once Associated, please submit any remaining data updates to the Ministers Directory Office as soon as possible, using the new Change Submission Tool in the Regional CDM+.  Be sure submissions include any specific changes to credentials.

Regions need to continue using DHM’s online Regional Directive form to allow clergy to use the S&C process, if the minister needs to do so before Standing is reported to DHM via Change Submission Tool.
