Clergy Continuing Education Application Regional Endorsement Form

The Grant Application Period is from September 1 to October 31.

This form is to be used by Regional Ministers (and/or designated Area/Associate Regional Ministers) to provide endorsements for, and as requested by, clergy with Standing in their region who are seeking a Clergy Continuing Education Grant from Disciples Home Missions. Clergy who are applying for their own grant should not complete this form. Upon submission, you (the regional minister) will receive a copy of this form via email. Thank you.
